29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  JOB SITE: 873-877 Broadwalk Springs Place, O Fallon, MO 63368, on or about 12/16/2015, a worker was observed using a pneumatic nail gun, actively install new roof shingles without any form of eye or face protection.  The worker was exposed to a struck-by hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures:  JOB SITE: 873-877 Broadwalk Springs Place, O Fallon, MO 63368, on or about 12/16/2015, workers were observed on top of a 6:12 pitch roof, approximately 19' 7" eave to ground level, actively tearing off old roofing material and installing new roof shingles, and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures.  The workers were exposed to a fall hazard.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  JOB SITE: 873-877 Broadwalk Springs Place, O Fallon, MO 63369, on or about 12/16/2015, workers were observed using a Werner Aluminum Extension Ladder, MODEL: D1224-2, 225lbs capacity, to gain up and down the roof and ladder side rails did not extend at 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  The workers were exposed to fall hazard.
