29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth:  (a) Within shoulder of westbound lane of East Juniper Street, approximately eighty-foot (~80') east of the intersection of East Juniper Street and Mill Street, Hazleton, PA: Employees working within a Type B soil trench without a safe means of egress were exposed to a caught-in hazard as observed on or about December 15, 2015.    In accordance with 29 CFR 1903.19(c), abatement certification is not required for this violation as the condition was corrected during the inspection.
