29 CFR 1910.28(a)(2): The footing or anchorage for scaffolds was not sound, rigid, and capable of carrying the maximum intended load without settling or displacement. Unstable objects such as barrels, boxes, loose brick, or concrete blocks are not to be used to support scaffolds or planks.  a) In the parking lot area, on December 10, 2015, the employer did not ensure that the elevated SkyJack (model 4626) aerial lift was properly footed on solid and stable grounding in that the aerial lift was on the elevated forks of a Yale powered industrial truck when the employee was changing a light bulb at an approximate height of thirty feet, exposing the employee to the hazards associated with death, paralysis, broken bones, internal injuries, lacerations, and bruises.

29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled:  a) In the parking area, on December 10, 2015, an employee elevated a SkyJack aerial lift (model number 4626) with a Yale powered industrial truck and subsequently raised the aerial lift creating an unstable load, exposing the employee to the hazards associated with falling loads.
