29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a. 1650 Spectrum Drive, Lawrenceville, GA (KCI of GA, Inc.) - Subcontractor employees (Roofers) were exposed to fall hazard. Subcontractor roofer employees were working (leaning over) and walking in close proximity to the edge of the flat roof finishing up the roof trim (roof term bar) and gutter work, approximately 16-18 feet above ground. The employees were not utilizing any type of fall protection, on or about 12/16/15.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: a. 1650 Spectrum Drive, Lawrenceville, GA (KCI of GA, Inc.) - Employees were exposed to fall hazard. Employees were using a 24ft. portable aluminum extension ladder that did not extend 3 feet above the upper landing while accessing the ladder to get to the roof of the commercial business, 12/16/15.
