Pioneer, Ohio; On or about December 12, 2015 the employer failed to ensure affected employees practiced making permit space rescues at least once every twelve months, by means of simulated rescue operations. Employees entering permit required confined spaces such as but not limited to the line 4 dip tank line of the finishing department for activities such as, but not limited to, cleaning are exposed to chemicals such as but not limited to potassium hydroxide, phosphoric acid, hydrofluoric acid, nickel fluoride, and sodium hydroxide.
