29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, and not installed under the supervision of a qualified person: On or about 12/17/2015, at the above addressed job site, two employees working on a roof were attached to an anchorage installed on an adjacent wall, which the anchorage was not installed under the supervision of a qualified person.
