OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by or crushed by hazards when operating a stand-up, open cab forklift truck that collides with storage racks: Warehouses: Employees who operate stand-up narrow aisle reach trucks were exposed to crushed by hazards. On or about December 9, 2015, an employee who was operating a stand-up reach truck struck a storage rack and was fatally crushed between the cross beam of the first shelf and the reach truck.
