29 CFR 1910.147(c)(5)(ii)(C)(1): Lockout devices were not substantial enough to prevent removal without the use of excessive force or unusual techniques, such as with the use of bolt cutters or other metal cutting tools: (a) On or about 12/17/2015 - Motor Control Center for Line 1, Picking Room, a Master Lock hasp designed to prevent access to an electrical cabinet inside the Motor Control Center (MCC) was damaged to the point that it could be removed from the mechanism with little effort, allowing for direct access to the breakers that are turned off.

29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished: (a) On or about 12/17/2015 - Line 1, Picking Room, an employee designated to clean the Quill Puller Machine did not ensure that the machine's energy source had been deenergized and that the machine was turned off prior to beginning the breakdown for cleaning.
