On or about 1/2/2020, at the job site located at 898 Barber Street, Athens, Georgia, the employer did not ensure designated Authorized employees received adequate training in the documented lockout procedures in that employees did not understand how to verify a machine was at a zero energy state prior to performing sanitation activities to equipment.
