29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. (a) NPC International dba Pizza Hut, 1201 N. Grant Ave, Springfield, Missouri did not report a hospitalization as a result of a work-related incident to OSHA within twenty-four hours. An employee was injured in an incident that occurred on or about December 10, 2015 which involved a hospitalization and the incident was not reported to OSHA until December 15, 2015.
