Each employer shall be responsible for the safe condition of tools and equipment used by employees, including tools and equipment which may be furnished by employees. a) 6444 Carlisle Pike, Mechanicsburg PA - On or about December 9, 2015, the employer did not ensure that a Werner 12 foot fiberglass stepladder (Model 3W422) was inspected and removed from service when needed, exposing employees to a fall hazard. Both spreader bars were bent and loose. No abatement certification or documentation required.

29 CFR 1910.303(b)(1): Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined using the following considerations: a) 6444 Carlisle Pike, Mechanicsburg PA; Upper Stockroom - On or about December 22, 2015, fluorescent lights located within several inches above cardboard boxes of merchandise and clothes racks were not protected from accidental contact or damage. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

When an authorized government representative asks for the records you keep under Part 1904, you must provide copies of the records within four (4) business hours. 6444 Carlisle Pike, Mechanicsburg PA - On or about December 22, 2015, an OSHA Form 301 or equivalent for the following work-related injuries or illnesses was not filled out or incorrectly filled for the calendar year 2015: a) The employer reported an "other recordable case" for a strained knee injury occurring on or about November 20, 2015 (case no. 301549270200001) and the OSHA 301 equivalent form did not contain the employees gender; name of physician or other health care professional; the location of where treatment was given; if the employee was treated in an emergency room or kept overnight for an in-patient hospitalization; the time the employee began work; and information regarding the object or substance that directly harmed the employee. b) The employer reported an "other recordable case" for a puncture to the foot occurring on or about November 25, 2015 (case no. 301549483010001) and the OSHA 301 equivalent form did not contain the employees gender; name of physician or other health care professional; the location of where treatment was given; if the employee was treated in an emergency room or kept overnight for an in-patient hospitalization; the time the employee began work; and information regarding the object or substance that directly harmed the employee. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
