29 CFR 1910.269(d)(6)(iii): All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from energy sources. On or about December 21, 2015, at the jobsite located at 25155 E. 2200 Rd., LaCygne, Kansas, the employer did not physically locate and operate the energy isolating device necessary to isolate the machine or equipment from an energy source (cooling water) prior to performing maintenance work.
