29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) On or about February 17, 2015, the employer failed to ensure that the paper cutting portions of bacon slicing lines #12, #13, and #14 were effectively guarded to prevent employee contact with the rotating blade shafts. View holes measuring approximately 2 x 2 inches did not contain barrier guards. Additionally, openings on both the input and output sides of the paper cutting equipment were too large allowing employees to reach the blade area.     b) On or about February 17, 2015, the employer failed to ensure that the side-strap chute portions of bacon slicing lines #12, #13, and #14 were effectively guarded to prevent employee contact with the rotating blades. The blade area was approximately 6.5 inches from the operator area. The lower portion of the barrier guard did not extend down to include coverage of the blade area.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:    On about December 17, 2015, the employer failed to ensure that the motor driven gears and sprockets of the cashin slicers of lines #12, 13, and 14 were effectively guarded to prevent employee exposure. The barrier guards could be easily removed and not affixed in place.
