29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) covered the following elements, however, the actions were not done in sequence as required by 29 CFR 1910.147 (d)(1)-(6): On December 22, 2015, an employee received a hand injury while unjamming a cardboard/glue jam in the Padlocker Doughboy Tray Maker and the employer did not ensure the employee performed the following activities prior to conducting unjamming activities: (1) Physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source; (2) Affix lockout or tagout devices to each energy isolating device; (3) Prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergization of the machine or equipment have been accomplished. Employees were required to rely on an ineffective, alternative safety measure such as, but not limited to, the machine's electronic control components while being exposed to caught-in and crushing hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
