OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees; in that employee were exposed to crushed-by hazards. (a) Warehouse- Bibro Building, 565 Coal Valley Road, Jefferson Hills, PA: On or about December 30, 2015, the industrial storage racks A007C and A009C were damaged, causing structural integrity issues that may cause a collapse of the racking system. Employees were exposed to crush-by hazards from the damage racking system.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: (a) 565 Coal Valley Road, Jefferson Hills, PA: On or about December 30, 2015, all employees that operate powered industrial trucks, such as but not limited to the motorized pallet jacks, were not provided with refresher training and evaluated at least once every three years.
