29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:   On or about December 30, 2015 during the installation of roofing material at the job site located at 6075 Upland Ridge Dr, Medina, Ohio 44256; the employer did not ensure employees working on the ground directly below multiple roofers removing and installing roofing material 12.3 feet above their location were protected from possible danger of head injury from impact, or from falling or flying objects from the roofing crew working directly above on the roof.
