29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. WORKSHOP: The ingoing nip points, rotating pulleys and gears of the following converting machines were unguarded: 1. The operator and the other sides of the bag handle tab and the rotating gears of the Converting Machine #3; unknown identification were unguarded. 2. The operator side of the bag handle tabs and the gears of the Converting Machine #4; unknown identification were unguarded. 3. The gears of the Converting Machine #5; unknown identification were unguarded. 4. The operator side of the bag handle tabs of the Converting Machine #1; unknown identification was unguarded.

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. WORKSHOP: The employees, as authorized, affected and/or other types were not provided with training and information in the controlling of potentially hazardous and unexpected releases of revised energy sources (lo/to) during servicing and maintenance of equipment, such as (but not limited to) converting machines. Flexo Converters USA, Inc. was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1910.147(c)(7)(i), which was contained in OSHA Inspection Number 1060362, Citation 1, Item Number 4, issued on 09/15/2015, with a final order date of 10-09-2015, with respect to a workplace location at 1200 Northrop Road, Meriden, CT 06450.
