On or about 5/23/2018, at 690 Unisa Drive, Monroe, GA: The employer failed to report to OSHA a work-related hospitalization and amputation within 24 hours of such knowledge and did not contact OSHA of the following injury:      4/3/2018, Tulsie Bhoodai, the Machine Operator was hospitalized with laceration and fractures to his left hand, pinky, ring, and middle finger.  OSHA became aware of this employees injuries during the inspection opened on 5/17/2018.
