29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      The employer did not ensure portable ladders were constructed and placed so three (3) feet or more was extended above the upper landing.  Employees performing residential roofing operations were exposed to falling approximately 11.7 feet while using an extension ladder to ascend/descend to the roof.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:    A 32-ft Werner extension ladder was not being used for the purpose for which it was designed. The base section and fly section of the ladder were separated and the fly section was used to access the roof of a residential house.

Madison County Roofing and Home Improvement Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.20(b)(2+), which was contained in OSHA inspection number 909966, citation number 2, item number 1 and was affirmed as a final order on 9/18/2013, with respect to a workplace located at 6956 Magona Ct. Maryville, IL 62062.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:      The employer did not ensure fall protection was implemented for employees performing residential roofing operations who were exposed to the hazard of falling approximately 11.7 feet.
