29 CFR 1910.37(a)(3): Materials or equipment were placed, either permanently or temporarily, within the exit route: On or about January 6, 2016, a bollard post with a missing cover (sleeve) was permanently fixed outside of an exit door path, exposing employees to trip hazard in case of an emergency. Employees move in and out of the warehouse to either bring in or take out materials.

29 CFR 1910.176(b): Storage was not secure. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. In the warehouse, on or about January 6, 2016, and at times prior to, pallets/materials were leaning, exposing employees to struck-by hazard. Employees move in and out of the warehouse to either bring in or take out materials.
