29 CFR 1926.20(b)(3): Machinery, tool(s), material(s), or equipment which was not in compliance with the applicable requirement(s) of 29 CFR 1926 was not prohibited, and/or was not either identified as unsafe by tagging, or locking the controls to render them inoperable, or physically removed from its place of operation. a) 800 Zorn Ave (Louisville, KY) - On or about 1/4/2015, employees were exposed to falling hazards when a damaged ladder was not tagged out. b) 800 Zorn Ave (Louisville, KY) - On or about 1/4/2015, employees were exposed to electrical hazards when a DeWalt portable angle grinder was not properly spliced with exposed wires. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
