29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: 764 South Athol Rd, Athol, MA On 1/4/16 and at times prior, maintenance technicians operated cutter machines with guards removed and with no alternate protection in place while troubleshooting belt alignment problems. On 1/4/16, one of the two maintenance employees troubleshooting a belt/pulley problem received a partial finger amputation injury.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: 764 South Athol Rd, Athol, MA As of 1/6/16, the employer had not performed an Energy Control Periodic Inspection for seventeen months. The Inspections previously performed did not include inspection of all Energy Control Procedures and evaluations/refreshers for all Authorized employees for all the energy control procedures they were authorized to use.
