29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: (a) Brundage-Bone Concrete Pumping, CSU Football Stadium, 615 W. Lake Street, Fort Collins, CO. 80521: On and prior to 12-21-2015, the employer had not provided sufficient instructions for pump truck operators to recognize the hazards associated with using web slings to vertically suspend tremie pipes from pump trucks. On 12-21-2015, a 50 foot long X 4 inch diameter tremie pipe was vertically suspended with a 1 inch wide web sling in a choke configuration. The web sling slipped off the end of the vertically suspended tremie pipe striking and seriously injuring a construction worker.

29 CFR 1926.702(g): Sections of tremies and similar concrete conveyances were not secured with wire rope (or equivalent material) in addition to the regular couplings or connections: (a) Brundage-Bone Concrete Pumping, CSU Football Stadium, 615 W. Lake Street, Fort Collins, CO. 80521: On or about 12-21-2015, a construction worker was struck by and seriously injured by a 50 foot long by 4 inch diameter tremie pipe after a 1 inch wide web sling used to elevate the pipe slipped off the end of the pipe. The regular couplings and connections consisting of chains, turnbuckles, and C clamp, were on-site but not used.
