Hickory Lakeside Subdivision, 4510 Palmetto Lane, Cumming, GA (Jobsite) - On or about July 10, 2021, Employees were exposed to struck-by and fall hazard. The employer did not perform frequent and regular inspections to ensure that the proper clean-out (cleaning) procedures, such as but not limited to the bucket sucking and/or wet sponge ball method, were performed in a safe and correct manner.

29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: a. On or about July 10, 2021, at Hickory Lakeside Subdivision, 4510 Palmetto Lane, Cumming, GA (Jobsite) -The employer failed to provide training and education related to safety requirements and hazard recognition while performing clean-up (cleaning) processes and/or concrete boom delivery truck, pipe and hose procedures. Employees were exposed to struck-by and fall hazards, associated with performing tasks, such as but not limited to the bucket sucking method and/or the wet sponge method.
