No abatement certification or documentation required for this violation.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:        a) On January 4, 2016, employees were exposed to eye injury while operating pneumatic nail guns without the eye protection during carpentry work.    W & K Construction Carpentry Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102 (a)(1) which was contained in OSHA inspection number 1011222, citation 1, item 1, and was affirmed as a final order on January 7, 2015, with respect to a worksite located at 1819 Elmwood, Wilmette, Illinois.          In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):              a) On January 4, 2016, employees were exposed to the hazard of falling approximately 25 feet while performing carpentry work from a second story deck without the use of fall protection.    b) On January 4, 2016, employees were exposed to the hazard of falling approximately 15 feet from an unprotected second story window opening while performing carpentry work without the use of fall protection.      W & K Construction Carpentry Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1011222, citation 2, item 1, and was affirmed as a final order on January 7, 2015, with respect to a worksite located at 1819 Elmwood, Wilmette, Illinois.      W & K Construction Carpentry Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 329355, citation 2, item 1, and was affirmed as a final order on May 30, 2012, with respect to a worksite located at 908 Beverly, Arlington Heights , Illinois.       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
