29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:         a) On November 30, 2017, employees were exposed to eye injury while operating pneumatic nail guns without the eye protection during carpentry work.     W & K Construction Carpentry Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102 (a)(1) which was contained in OSHA inspection number 1116331, citation 2, item 1, and was affirmed as a final order on May 5, 2016, with respect to a worksite located at 41 Birchwood, Northbrook, Illinois.     W & K Construction Carpentry Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102 (a)(1) which was contained in OSHA inspection number 1011222, citation 1, item 1, and was affirmed as a final order on January 7, 2015, with respect to a worksite located at 1819 Elmwood, Wilmette, Illinois.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):      a) On November 30, 2017, employees were exposed to the hazard of falling while working without fall protection from a pitched roof having a peak height of approximately 17 feet and a ground to eave height of between 9 and 10 feet.     W & K Construction Carpentry Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1141227, citation 1, item 1, and was affirmed as a final order on May 5, 2016, with respect to a worksite located at Lot #4 Wilmette Circle, Wilmette, Illinois.      W & K Construction Carpentry Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1116331, citation 2, item 2, and was affirmed as a final order on May 5, 2016, with respect to a worksite located at 41 Birchwood, Northbrook, Illinois.
