29 CFR 1910.178(l)(4)(ii)(E): Refresher training was not provided to forklift operators after a condition changed that affected safe operation of the truck:  On or about December 29, 2015:  a) Employer failed to properly provide refresher training to powered industrial truck drivers, when the employer installed a new loading dock area, which had a drop off of approximately 3.8 feet, with no edge protection installed.
