29 CFR 1910.94(a)(6): The air for abrasive-blasting respirators was not  free of harmful quantities of dusts, mists, or noxious gases, and did not meet the requirements for air purity set forth in 29 CFR 1910.134(i):  Blasting Booth:  On or about 1/7/2016, and times prior thereto; employees were operating an Air Systems International, Model BAC-17, Serial # 68985, air supply system to provide breathing air for employees conducting abrasive blasting operations.  The breathing air supply system did not have a Carbon Monoxide alarm and testing was not conducted where vehicle emissions were present to ensure exposure was less than 10 parts per million.  In addition, the frequency of air quality testing for Grade D air had not been determined at the time of the inspection as required by 1910.134(i).

29 CFR 1910.1200(e)(1)(i): The employer did not comply a list of the hazardous chemicals known to be present using a product identifier that was referenced on the appropriate safety data sheet.  Throughout the Facility, on or about 07 January 2016 and times prior; the employer did not have a comprehensive list of hazardous chemicals known to be present using an identity that is referenced on the appropriate Safety Data Sheets (SDS) available. This condition exposed employees to delays in readily accessing SDS information.

29 CFR 1910.1200(f)(5): Chemical manufacturers, importers, or distributors did not ensure that each container of hazardous chemicals leaving the workplace was labeled, tagged, or marked in accordance with 29 CFR 1910.1200.  Shop area; on or about 07 January 2016 and times prior thereto; three instances were noted were unmarked plastic spray bottles, containing an unknown chemical substance were not marked identifying the product and the hazard warnings associated with it:       a. In the fire room; two plastic two gallon spray bottles were not marked indicating their chemical content            b. In the shop bay adjacent to the sink, a 32ounce plastic spray bottle was not marked indicating the chemical content.  This condition exposed employees to chemical burns, poisoning, and skin dermatitis.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information.  Throughout the worksite; on or about 07 January 2016 and times prior thereto; employees were not trained on the new Hazard Communications Standard (HCS 2012) provisions implemented on 1 December 2013.This training includes revised labeling elements and the new format for Safety Data Sheets. This condition exposed employees to inadequate training and chemical hazards.
