29 CFR  1917.26(c):  The contents of each first aid kit were not checked at intervals that allowed prompt replacement of expended items.  a) Maintenance Shop:  on or about May 19, 2023, there was expired antibacterial solution dated 2007 in the first aid kit along with several other containers of expired products dated 2009, exposed employees to biological hazards and ineffective products.  b) Maintenance Shop:  on or about May 19, 2023, the AED had expired shock pads dated 2019, exposing employees to limited medical first aid.  c) Maintenance Shop:  on or about May 19, 2023, the eyewash bottles had expired fluid, one bottle was dated 2016 and the other 2017 which exposed employees to biological or irritation hazards.

29 CFR  1918.105(a)(1):  The employer did not provide special protective clothing for each employee engaged in work where protective clothing was necessary.  a) Valdez terminal dock:  On or about May 19, 2023, employees assisting in the removal of equipment from one vessel were on the dock wearing PFDs; however only 1 device was bright orange the rest did not have reflective markings or a reflective vest when a fork truck and a crane were in operation nearby, exposing them to struck by hazards.
