29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources. Throughout Facility - On or about January 7, 2016 the employer failed to provide the use of safety blocks for employee use during the installation and removal of dies into presses thereby exposing employees to crushing and amputation type injuries.

29 CFR 1910.147(f)(1)(ii): Testing or positioning of machines, equipment or components thereof. In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the following sequence of actions shall be followed: Remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section; Throughout Facility - On or about January 4, 2016, the employer failed to remove employees from the danger zone when testing or positioning a die into the Danly press no. U4-1000-144x90, Serial #55093301-12, thereby exposing employees to amputation and crushing type injuries.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained. Throughout Facility - On or about January 4, 2016, personal lockout or tagout devices were not attached to a group lockout device on the Danly press number U4-1000-144x90, serial number 55093301-12 when four employees were exchanging, adjusting and tying in dies, thereby exposing employees to the hazards of the unexpected re-energization of equipment.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. Throughout Facility - On or about January 7, 2016, the employer failed to guard the point of operation when exchanging dies and making adjustments causing employees to be positioned in the danger zone within the cell, thereby exposing employees to amputation and crushing injuries.
