29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:   (a) West side gable, new residential construction located at 12713 Ruth's Farm Way, Alpharetta, GA 30009: Employees were exposed to fall hazards of approximately 20-feet, 7-inches while engaged in siding activities on a ladder jack scaffold platform with no fall protection system in place.   Elvin Chirinos was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(vii), which was contained in OSHA inspection number 1103394, citation number 1, item number 1 and was affirmed as a final order on 12/01/2015, with respect to a workplace located at 409 Hawthorne Avenue, Smyrna, GA 30080.  Elvin Chirinos was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 925011, citation number 1, item number 1 and was affirmed as a final order on 09/25/2013, with respect to a workplace located at 1717 Whitfield Circle, Smyrna, GA 30080.

29 CFR 1926.452(k)(1): Platforms shall not exceed a height of 20 feet (6.1 m):  (a) West gable wall, new residential construction located at 12713 Ruth's Farm Way, Alpharetta, GA 30009: The employer failed to ensure that the height of the ladder jack platform did not exceed 20-feet while employees were engaged in siding installation activities. Employees were exposed to collapse/fall hazards while working at heights of approximately 20-feet, 7-inches on a ladder jack scaffold with no fall protection system in place.    Elvin Chirinos was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.452(k)(1), which was contained in OSHA inspection number 981457, citation number 1, item number 1 and was affirmed as a final order on 12/03/2014, with respect to a workplace located at 2708 Three House Parkway, Norcross, GA 30093.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Garage roof, new residential construction located at 12713 Ruth's Farm Way, Alpharetta, GA 30009: Employees were exposed to falls while working from a 10/12 pitched roof while accessing the ladder jack scaffold which was attached to the wood 2x4 cleats and the ridge. Employees working from this roof were exposed to falls of approximately 11-feet, 8-inches to the lower level as a fall protection system was not provided.   (b) Porch roof, new residential construction located at 12713 Ruth's Farm Way, Alpharetta, GA 30009: Employees were exposed to falls of approximately 11-feet, 8-inches to the lower level while working on the porch roof and the roof directly above with no fall protection system in place.   Elvin Chirinos was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 994175, citation number 1, item number 1 and was affirmed as a final order on 12/03/2014, with respect to a workplace located at the corner of Redwine Street & Gaines Street, Norcross, GA 30071.
