29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written respiratory protection program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use did not present a health hazard to the user:  (a) 148 Sheraton Drive, New Cumberland, Pennsylvania - The employer permitted maintenance employees to voluntarily wear North 5500 series elastomeric half-mask air-purifying respirators with N75001L organic vapor cartridges when the employees did maintenance activities such as painting and investigating water leaks.  The employer did not develop and implement any written respiratory protection program, exposing the employee to hazards of inhalation, ingestion, or absorption of contaminants and increased physiological burden, on or about January 8, 2016.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) Employee Break Room by the Sales Office - The employer used a relocatable power tap (a.k.a., a power strip or surge protector) to power a Haier compact refrigerator.  The UL listing for the power tap specifies use in supplying power to low amperage electronic equipment rather than appliances such as the refrigerator, exposing employees to an electrical hazard, on or about January 8, 2016.  (b) Pool Equipment Closet - A metal junction box, with knockouts on the sides, which supplied electrical wiring to the Hayward pool pump, model K48M2N111, laid on the floor.  The employer did not have this metal knockout box installed in accordance with the box's listing, labeling, or certification in that the employer did not have this knockout box securely and rigidly mounted to the building or other support structure, exposing employees to an electrical hazard, on or about January 8, 2016. Corrected during the inspection.  (b) Pool Equipment Closet - The door of the enclosure box for the Intermatic T100 series mechanical timer switch remained open, exposing employees to an electrical hazard, on or about January 8, 2016.  Corrected during the inspection.  (c) Laundry - The Huebsch Originators gas drying tumbler and the Cissell gas dryer (right next to the Huebsch drying tumbler) both operated with the access panel doors, located on the top front of these two dryers, open, exposing live wiring and thereby exposing employees to an electrical hazard, on or about January 8, 2016.  Abatement certification required within 10 days after abatement date.
