29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times:   A.  On or about January 12, 2016, Front of shop, near electrical panel:  A side-hinged door provided with an illuminated exit sign had the door handle removed.  The provided emergency exit was not maintained in working condition.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  A.  On, or about January 12, 2016, Shop Floor:  Employees routinely unjammed the SSI Model M85E Shredder without locking out the machine.   The on/off control for the conveyor was an unguarded snap switch that was not protected against accidental contact.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items (a) through (d) of this section:  A.  On, or about, January 12,2016, Shop Floor:  Employees routinely unjammed the SSI Model M85E Shredder without locking out the machine. The lockout procedures used did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  A specific statement of the intended use of the procedure; Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  A.  On or about January 12, 2016, Conveyor:  At the payoff end of the conveyor near the SSI Model M85E Shredder, a junction box had an open knockout on the top left side, an open knockout on the top right side and a faceplate that was dislodged. Employees accessing the shredder unit from the adjacent metal industrial stairs were exposed to electric shock hazards.

On or about January 12, 2016, Mattress Storage Area:  Access to the emergency exit in the southeast corner of the shop continued to be obstructed by the placement of mattresses and box springs. Access was reduced to 22 inches.   Recyc-Mattresses Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.37(a)(3)), which was contained in OSHA inspection number 1086374, citation number 1, item number 3 and was affirmed as a final order on (October 26, 2015), with respect to a workplace located at 12 Eastern Park Road, East Hartford, Connecticut.

29 CFR 1910.304(f)(1)(iv): Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel:  A.  On or about January 12, 2016, Front of Shop:  The General Electric circuit breaker panel that provided power to machines in the shop such as balers, the shredder, the conveyor and battery chargers continued to be blocked.  Access to the panel was blocked by metal bushings, cardboard and plastic wrap.   Recyc-Mattresses Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.304(f)(1)(iv)), which was contained in OSHA inspection number 1086374, citation number 1, item number 14 and was affirmed as a final order on October 26, 2015, with respect to a workplace located at 12 Eastern Park Road, East Hartford, Connecticut.
