29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:  A.  On or about July 25, 2016,  Warehouse:  The employer did not ensure that each powered industrial truck operator was provided practical training on the two Toyota Electric Forklift Trucks operated on site.

On or about July 25, 2016, Warehouse:  Powered industrial truck training for employee(s) had not been documented through certification.  The employer did not maintain records which included the name of the operator, date of training, date of evaluation or identify of the person performing the training or evaluation.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:  A.     On or about July 25, 2016, Mattress Storage Area:  Access to the emergency exit in the southeast corner of the shop continued to be obstructed by the placement of mattresses and box springs.  A pile of mattresses had toppled, complete blocking access to the exit door.  B.     On or about July 25, 2016, Front Loading Area:  Access to the exit route along the right side of the building was blocked by the placement of a box spring, excess plastic sheeting and a pallet holding a Coleman Powermate Model XL 5006016 air compressor.    Recyc-Mattresses Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.37(a)(3)), which was contained in OSHA inspection number 1086374, Serious Citation Number 1, Item Number 3 and was affirmed as a final order on October 26, 2015, with respect to a workplace located at 12 Eastern Park Road, East Hartford, Connecticut.    Recyc-Mattresses Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.37(a)(3)), which was contained in OSHA inspection number 116974, Repeat Citation Number 2, Item Number 1 and was affirmed as a final order on July 19, 2016, with respect to a workplace located at 12 Eastern Park Road, East Hartford, Connecticut.
