On or about January 11, 2016, at the southwest side of building #9, two employees picking up debris on the ground were not wearing hard hats while work was being done above them, exposing themselves to head injuries. b. On or about January 11, 2016, at the east side of building #8, two employees picking up debris on the ground were not wearing hard hats while work was being done above them, exposing themselves to head injuries.

On or about January 11, 2016, at the south side of building #9, five employees removing old weather proofing material on a 10:12 pitch residential roof were not protected from a 27 feet fall hazard by the use of a fall protection system. b. On or about January 11, 2016, at the east and west side of building #8, three employees installing weather proofing material on a 10:12 pitch residential roof were not protected from a 27 feet fall hazard by the use of a fall protection system.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in mounting the ladder: a. On or about January 11, 2016, at the west side of building #8, the Werner 12-foot aluminum ladder placed on the canopy roof only extended 6 inches above the landing surface and it was not secured at the top nor there was a grab-rail, exposing employees to a 12ft-9in fall hazard to the ground.
