29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about 08/21/2018, at the job site - employees were exposed to a 27 foot fall hazard while conducting roofing work on a 5 in 12 pitched roof, in that, fall protection was not used.  INTEGRITY ROOFING & GUTTERS INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(B)(10), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1257467, CITATION NUMBER 01, ITEM NUMBER 001 AND WAS AFFIRMED AS A FINAL ORDER ON 01/30/2018, WITH RESPECT TO A WORKPLACE LOCATED AT 5933 LEE VISTA BLVD., ORLANDO, FL 32822.    INTEGRITY ROOFING & GUTTERS INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(B)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1128069, CITATION NUMBER 01, ITEM NUMBER 001 AND WAS AFFIRMED AS A FINAL ORDER ON 06/14/2016, WITH RESPECT TO A WORKPLACE LOCATED AT 13000 ISLAND BAY CIRCLE SANFORD, FL 32771. On or about 08/21/2018, at the job site - The employer did not maintain fall protection training certifications for employees exposed to 27 feet fall hazards while conducting roofing work on a 5 in 12 pitched roof.
