OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushed-by hazards while using automotive lifts that were not maintained and inspected.  a) Garage One, Bay 4, 1101 Fourth Avenue, Coraopolis, PA 15108:  Observed on or about January 12, 2016, a Rotary brand two post inground automotive lift was that was leaking hydraulic oil and exhibiting signs of erratic behavior was not immediately taken out service and repaired by a qualified person.  Employees using the automotive lift were exposed to struck-by and crushed-by hazards.  b) Garages One and Two, all bays, 1101 Fourth Avenue, Coraopolis, PA 15108:  Observed on or about January 12, 2016, automotive lifts in both garages were not inspected by a qualified lift inspector at a frequency required by the lift manufacturer or at least annually.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of hearing loss and being struck-by fragments of polyvinyl chloride (PVC) plastic pipe in the event the pipe ruptured:  a) Garage Two, 1101 Fourth Avenue, Coraopolis, PA 15108:  Observed on or about January 12, 2016, PVC plastic piping was used in exposed above ground areas to transport compressed air at approximately 130 pounds per square inch (p.s.i.).

29 CFR 1910.215(a)(1): Abrasive wheel(s) used on grinding machinery were not used with safety guard(s):      a) Garage One, near Bay 4, 1101 Fourth Avenue, Coraopolis, PA 15108:  Observed on or about January 12, 2016, the grinding wheel on the right side of a Clarke brand bench grinder was not equipped with a tongue guard.  Employees using the bench grinder were exposed to a fire hazard and struck-by hazards created by the lack of a tongue guard.  b) Garage Two, south wall, 1101 Fourth Avenue, Coraopolis, PA 15108:  Observed on or about January 12, 2016, the grinding wheel on the right side of a Sunex Tools brand bench grinder (model number MD200KL) was not equipped with a tongue guard.  Employees using the bench grinder were exposed to a fire hazard and struck-by hazards created by the lack of a tongue guard.

29 CFR 1910.215(a)(4):  Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:   a) Garage One, near Bay 4, 1101 Fourth Avenue, Coraopolis, PA 15108:  Observed on or about January 12, 2016, the grinding wheel on the right side of a Clarke brand bench grinder was not equipped with a work rest.  Employees using the bench grinder were exposed to a caught-by hazard.    b) Garage Two, south wall, 1101 Fourth Avenue, Coraopolis, PA 15108:  Observed on or about January 12, 2016, the work rest on the grinding wheel on the right side of a Sunex Tools brand bench grinder (model number MD200KL) was not adjusted to within 1/8th of an inch from the grinding wheel.  The gap between the work rest and the grinding wheel was approximately 1 and 1/4th inches.  Employees using the bench grinder were exposed to a caught-by hazard.

29 CFR 1910.305(a)(2)(ix):  All lamps for general illumination were not protected from accidental contact or breakage by a suitable fixture or lampholder with a guard:                                     a) Body Shop, Garage Two, 1101 Fourth Avenue, Coraopolis, PA 15108:  Observed on or about January 12, 2016, the light bulbs in light fixtures on the walls of the body shop were not protected from accidental contact or breakage by a suitable fixture or lampholder with a guard.  The light bulbs fixtures were located approximately 4 feet above the body shop floor.  Employees working in the body shop were exposed to the hazards of electric shock and cut-by hazards from broken glass in the event that the light bulbs would break.
