29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means:   On January 9, 2016 at approximately 14:45 the employer failed to protect employees against explosion and fire hazards associated with horizontal boring work when the employer did not identify the gas line feeding into the business located at 422 South 11th Street in Omaha, NE.  The employers failure to pot hole the ground near the markings left by the municipal utility company created the risk of the Ditch Witch JT20 striking the gas line while on the return run from the bore they just drilled; thus  resulting in a gas leak that seeped back into the business and caused an explosion and fire that destroyed the structure and endangered the lives of the employees of both the business and the contractor.
