Section 5(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious harm to employees in that employees were exposed to struck by:  (a)	On or about October 24, 2022, in the vicinity of 21st Street and 138th Avenue outside of Page, North Dakota, for the employee exposed to being struck-by a RT115 Ditch Witch Quad Track while engaged in the installation of fiber optic cables.  Abatement Note:  Among other methods, feasible and acceptable means to correct this hazard include but are not limited to:   Following the RT115 Q Operators Manual which states; Allow only responsible, properly instructed individuals to operate machine; Keep all spectators and other workers away from the machine and work area while in operation.
