29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:  Employees accessing the storage area above the offices were exposed to fall hazards of approximately 10 feet to the concrete below.  The stair platform guardrail height only measured 29 1/2 inches high.

Open flame ignition sources were found in the following locations:  a)  Gas fired heater located in the northeast corner of the wash bay. b)  Gas fired heater located in the maintenance area along the north wall. c)  Oil fired heater located between the lube area and the truck maintenance area.

29 CFR 1910.307(c): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location:      On or about January 12, 2016 employees working in the facility were exposed to fire and explosion hazards.  The following electrical equipment was used in locations that have the potential for release and were not rated for use in hazardous locations:  a)  Electrical disconnects, switches and junction boxes used to operate the boiler mounted on the west wall of the wash bay. b)  Electrical disconnects and motor controllers to operate the pressure washer mounted on the west wall of the wash bay. c)  The two electric air compressors located along the west wall of the maintenance area next to the wash bay. d)  The electric motor used to pump motor oil located at the doorway between the wash bay and the maintenance area. e)  The extension cord affixed to the wall between the wash bay and maintenance area.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  In the maintenance area, a flexible cord used to power an encased plastic drop light was found to have a broken ground prong.
