At the workplace, a powered industrial truck trainee was not under direct supervision for the entire duration of the training period prior to having an evaluation completed, thereby exposing employees to a struck-by hazard. On or about January 4, 2016, a powered industrial truck trainee lost control of the truck while traveling behind the trainer with other powered industrial truck traffic, resulting in a broken leg from the accident.
