29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    a.  In the sawmill where employees were changing out blades on the saws, the employer did not develop a written lock out/tag out program which includes training for authorized and affected employees on the proper procedures for taking the equipment to a zero state thereby exposing employees to a contact hazard.

29 CFR 1910.213(i)(1): All portions of the saw blade shall be enclosed or guarded, except for the working portion of the blade between the bottom of the guide rolls and the table. Bandsaw wheels shall be fully encased. The outside periphery of the enclosure shall be solid. The front and back of the band wheels shall be either enclosed by solid material or by wire mesh or perforated metal. Such mesh or perforated metal shall be not less than 0.037 inch (U.S. Gage No. 20), and the openings shall be not greater than three-eighths inch. Solid material used for this purpose shall be of an equivalent strength and firmness. The guard for the portion of the blade between the sliding guide and the upper-saw-wheel guard shall protect the saw blade at the front and outer side. This portion of the guard shall be self-adjusting to raise and lower with the guide. The upper-wheel guard shall be made to conform to the travel of the saw on the wheel.  a.  In the sawmill where tree logs are cut into logs to be made into pallets, the blade was not covered or guarded on the unused portion of the blade thereby exposing employees to a contact hazard.

29 CFR 1910.219(e)(1)(i): Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section.    a.  In the Re Saw area where the Pendu Sizer Head is operated, there was an unguarded opening to a belt and pulley which measured 15 3/4 x 4 1/2 inches thereby exposing employees to a caught-in hazard.    b.  In the Re Saw Area, the Cut Off saw was missing a guard around the belt and pulley thereby exposing employees to a caught-in hazard.    c.  In the Pallet Building, the Morgan Chop Saw there was no guard or enclosure around the belt and pulley thereby exposing employees to a caught-in hazard.     d.  In the Pallet Building, there was a missing guard on the back of the air compressor which prevented indirect contact with the belt and pulley thereby exposing employees to a caught-in hazard.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.    a.  In the Re Saw area where logs were cut down and run through the Re Saw so that all the runners were the same size, there was an opening in the guard which measured 15 3/4 inches by 4 1/2 inches to the chain and sprocket thereby exposing employees to a caught-in hazard.

On the east wall of the Re Saw Building, the energized electrical disconnect for the waste conveyor feed roll had an unused opening on the left side of the feed roll disconnect thereby exposing employees to an electrical hazard.  b.  On the east wall of the Re Saw Building, the energized electrical disconnect for the Master Controls had an unused opening on the left side and bottom of the disconnect thereby exposing employees to an electrical hazard.  c.  On the east wall of the Re Saw Building, the energized electrical disconnect for the Infeed Unscambler had an unused opening on the left side and bottom of the disconnect thereby exposing employees to an electrical hazard.  d.  In the sawmill where boards were cut with the trim saw, there was an unused opening in the side of the trim saw disconnect thereby exposing employees to a electrical hazard.  e.  In the energized electrical panel which controls the lights in office, there were 2 missing circuit breakers thereby exposing employees to an electrical hazard.  f.  In the sawmill, the main electrical disconnect had an unused opening on the left and right side of the disconnect thereby exposing employees to an electrical hazard.  g.  In the sawmill, there was an unused opening in the front of the cut-off saw starter thereby exposing employees to an electrical hazard.  h.  In the sawmill, there was a unused opening on the left side of the Square D disconnect for the air compressor thereby exposing employees to an electrical hazard.  I.  In the Pallet Building, there was an unused opening in the bottom of the main electrical disconnect thereby exposing employees to an electrical hazard.
