29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: On or about January 14, 2016 at the Ozarks Plastics Products Inc. facility- employees were exposed to a struck by hazard from multiple 6" Bench Grinders, in the Maintenance Shop, and throughout the facility which had tongue guards that were improperly adjusted in excess of one-fourth inch. Abatement certification is not required for this item:

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about January 14, 2016 at the Ozarks Plastics Products Inc. facility- employees were exposed to a 460V Grinder that had a flexible cable that pulled loose from its attachment point to a portable outlet box leaving the cable attached to the device by its internal conductors. Abatement certification is not required for this item.
