29 CFR 1910.23(b)(1): Every wall opening from which there is a drop of more than 4 feet was not guarded in accordance with the requirements of 29 CFR 1910.23(b)(1)(i) or 29 CFR 1910.23(b)(1)(ii):    a) On or about June 8, 2016, the employer failed to guard a wall opening from which there is a drop of more than 4 feet. The dryer platform, which measured approximately 59.75 inches high, was not guarded at the opening in the railing over the fixed ladder. Employees were exposed to serious fall hazards.

An employee was observed to have sustained a second degree thermal burn on the hand where the employee's glove was threadbare.
