29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    The employer did not develop and implement a lockout tagout program consisting of energy control procedures and periodic inspections for employees performing servicing and maintenance work on equipment covered by the standard such as, but not limited to, The Run-A-Gade high volume band saw pallet disassembler, Viking Engineering & Development, Inc. Champion pallet machines, Rayco Industries, Inc. pallet machine, and Pendu Saw System M4300N.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a)  Employees operating the Run-A-GADE high volume band saw pallet disassembler located in the pallet breakdown room were exposed to the band saw while pulling the pallet back over the running blade.  On or about September 23, 2015, an employee received a partial amputation when their hand slipped off a pallet as it was being pulled over the running band saw blade.      (b)  Employees operating the two Viking Engineering & Development, Inc. Champion pallet machines located in the pallet assembly room were exposed pinch points created by the traveling carriage and the automatic hydraulic nail guns.
