29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways: (a) for subcontractor employees who were working at The Home Depot Inc. Warehouse located at 8423 Sunstate St. Tampa (FL) while using a Werner 16 foot fiberglass extension ladder fully extended and placed on top of a wooden box with a slide guard installed in front of its feet with a piece of wood behind to ensure that the ladder would not slip, on or about 01/20/2016.
