29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer is failing to protect employees from caught between hazards associated with operating the Carton Glue Packaging Machine #1. Specifically that on January 13, 2016 in the packaging department of the facility located at 9601 F Street, Omaha, NE; the operator for the Carton Glue Packaging Machine #1 was pinned in an unguarded space between the robotic actuated arm/picker and the frame of the machine while trying to remove a damaged slip before it caused the machine to jam.
