OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crush-by hazard: On or about 1/20/2016, at the above addressed jobsite, an employee was exposed to a crush hazard from a CROWND RR S700 Series Lift Truck while riding on the mast and suffered an amputation to part of his left foot.
