29 CFR 1910.24(b): Fixed stairs were not provided for access from one structure level to another where operations necessitated regular travel between levels, and for access to operating platforms at any equipment which requires attention routinely during operations: Employees engaged in maintenance and standard operation of Cooker #3 were exposed to slips and fall hazards up to 10 feet 6 inches to the lower level in that fixed stairs were not provided to access the platform.

29 CFR 1910.27(f): "Maintenance." All ladders shall be maintained in a safe condition. All ladders shall be inspected regularly, with the intervals between inspections being determined by use and exposure. Employees engaged in maintenance and standard operation of Cooker #3 were exposed to slips and fall hazards up to 10 feet 6 inches to the lower level in that the ladder leading up to the control wheel platform on the south end of Cooker #3 was not maintained in a safe condition.

29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates or markings were not maintained in a legible condition: The employer is failing to protect employees from struck by hazards when operating forklifts. This was most recently documented at 1501 E 8th S. Liberal, KS, 67901 where forklift markings were not maintained in a legible condition for the Yale Lift Truck, Model GLP040SVXNUSE084, Serial No. C810V04447J in the following instances: a) The directional controls were not legible. b) The load ratings were covered by a sticker. The National Beef was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(a)(6), which was contained in OSHA inspection number 480339, citation number 2, item number 1 and was affirmed as a final order on January 28, 2013, with respect to a workplace located at 2000 E. Trail, Dodge City, KS 67801.
