29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) At the job site, on or about January 20, 2016, employees exposed to overhead hazards were not wearing a hard hat, exposing employees to struck-by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) At the job site, Buildings #8, 9, 10 on or about January 20, 2016, several employees were performing framing work on a new two-story residence while exposed to fall hazards up to approximately 18 feet onto earthen ground because the workers were not fall protected.    b) At the job site, Buildings #11, 12 and 15 on or about January 20, 2016, several employees were performing framing work from the 2nd floor balcony while exposed to fall hazards up to 11 feet onto earthen ground because the workers were not fall protected.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:  a) At the job site, on or about January 20, 2016, three employees were using the top two steps of a 6-foot A-framed step ladder to exit the upper level platforms where the work was being performed, resulting in a fall hazard.
